Common side effects
The most common side effects are nausea, diarrhea, vomiting, constipation, stomach pain, and lower appetite. They are usually mild and ease as your body adjusts, and a low starting dose helps.
Serious risks to know
Serious but less common risks include inflammation of the pancreas, gallbladder problems, kidney strain from dehydration, low blood sugar (especially with insulin or a sulfonylurea), and allergic reactions. In animal studies this class caused thyroid tumors; the risk in people is not known, which is why the thyroid-history questions matter. Tell your provider about any severe belly pain, trouble breathing, or swelling.
Before you start
A provider will not prescribe tirzepatide if any of the following apply to you. Please be honest in your visit, these questions keep you safe.
- A personal or family history of medullary thyroid carcinoma (a type of thyroid cancer)
- Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
- Pregnant, breastfeeding, or planning to become pregnant
- A history of pancreatitis (inflammation of the pancreas)
- Type 1 diabetes, or a history of diabetic ketoacidosis
- A previous serious allergic reaction to semaglutide, tirzepatide, or any GLP-1 medication
- Severe stomach or intestinal disease, such as Crohn's disease, ulcerative colitis, or gastroparesis (slow stomach emptying)
- Active gallbladder disease
- Bariatric or other stomach or intestinal surgery in the last 6 months
- Liver disease or cirrhosis
- Kidney disease or a history of kidney failure
